Cause of Wales train crash which killed man and injured 15 is revealed

An automated system designed to help train wheels grip tracks failed on a train involved in a fatal crash in mid-Wales, according to the Rail Accident Investigation Branch (RAIB). The crash resulted in one passenger death, four serious injuries, and 11 others needing hospital treatment. The investigation revealed that the system’s sanding hoses were blocked, preventing the discharge of sand to increase friction between the wheels and tracks.
