The health regulator imposed these conditions after investigating the incident, which the company attributed to human error and IT issues. This incident follows a similar case where a woman in Brisbane received another patient’s embryo, leading to an independent review. Monash IVF stated that they have already implemented necessary process changes and do not expect these conditions to significantly impact operations.
