WorkSafe alleges South32 negligence in death of Simon Mukwarami

A father, Simon Mukwarami, tragically died at South32’s Worsley Alumina refinery in Western Australia after falling through a grid mesh walkway on March 14. A leaked WorkSafe notice revealed that no fall prevention measures were in place at the site, including the lack of fall-arrest equipment.
