The National Transportation Safety Board (NTSB) will hold its final meeting on Tuesday regarding the midair collision between a U.S. Army Black Hawk helicopter and an American Airlines passenger jet that resulted in 67 fatalities nearly a year ago. NTSB Chair Jennifer Homendy indicated that while the investigation revealed multiple systemic failures, no single cause will be identified. The final report will include an extensive list of safety recommendations. The U.S. government recently acknowledged partial fault in the incident due to the helicopter crew’s failure to maintain safe visual separation from the jet.
