A 178-page report on Greece’s deadliest train crash blamed human error, outdated infrastructure, and systemic failures for the collision that killed 57 people. The investigation found a routing mistake by a station master led to a passenger train colliding with a freight train, citing poor training, staff shortages, and a lack of automated safety controls in the deteriorating railway system. The findings were released by the Hellenic Air and Rail Safety Investigation Authority ahead of a general strike and mass protests marking the anniversary of the 2023 crash.
