NTSB Chair Jennifer Homendy indicated that while the investigation revealed multiple systemic failures, no single cause will be identified. The final report will include an extensive list of safety recommendations. The U.S. government recently acknowledged partial fault in the incident due to the helicopter crew’s failure to maintain safe visual separation from the jet.
