Kilvington Grammar School in Melbourne was fined $140,000 after student Lachlan Cook, 16, died from diabetes complications during a school trip to Vietnam in 2019. Lachlan, who was self-managing his type-1 diabetes, fell ill and suffered a heart attack, leading to his death in a Melbourne hospital. The school and travel company World Challenge Expeditions were found guilty of failing to ensure non-employees were not exposed to health and safety risks, with both entities receiving fines and convictions.
