Report identifies issues in Lisbon funicular crash

A preliminary report on last month’s funicular crash in Lisbon, which killed 16 people, including three Britons, attributed the tragedy to a faulty steel cable and significant maintenance failures. The report noted that the cable, not certified for public transport, caused safety systems to fail, leading to the carriage derailing and crashing into a building.
