The Ministry of Social and Family Development found that the social service agency involved in Megan Khung’s care did not fully describe the severity of her injuries, leading to inadequate interventions before her death. Megan, who was abused by her mother and her boyfriend, was withdrawn from preschool months before her death. Despite bruises being noticed on her body, her death only came to light five months later. The report highlighted that a temporary care plan was established for Megan, and she was supposed to move in with her grandmother for her safety.
