Johnathon Taituma, a 43-year-old man from Auckland, struggled to breathe and called 111 for assistance, but a St John call taker assigned the wrong priority code, preventing an ambulance from being dispatched. His neighbor also called 111, but again, the incorrect code was entered. An hour and a half later, Taituma was found unconscious, and by the time help arrived, it was too late. A coroner’s report indicated that he might have survived if not for St John’s “cascade of errors.”
