NHS trust fined £1.6m over failings in baby deaths

Adele O’Sullivan, Kahlani Rawson, and Quinn Parker died shortly after birth, with the trust admitting six counts of failing to provide safe care and treatment. The trust was also ordered to cover prosecution costs and pay a surcharge, with the judge emphasizing the gravity of the offenses and the need to hold the trust accountable for their failures.
